Below are a variety of important forms that you may need to access and complete throughout the school year.

Parent Information from Nurse  Office hours, permissions, immunization requirements, etc.
Emergency Medical Authorization  Required to authorize treatment for students while at school
General Medication Form  Required for students who require Inhalers or Epipens at school


Registration Form   Required to enroll new student for the 2021-2022 school year
Re-Registration 2021-2022  Required to re-enroll students for the 2021-2022 school year
Health History  Required for registration
Legal Custody  Required for registration - update when any changes

 To be reviewed by students and parents before school begin


Internet Policy and Access Agreement  All parents and students must sign annually (after reading policy)


Communication Permissions  Parents sign to authorize school to contact you by email, text message and photo permissions
Emergency Dismissal   Emergency contact form.


Parent/Student Handbook  To be reviewed annually by parents and students
Code of Conduct  To be reviewed annually and signed by parents and students
